Diagnostic Evaluation

Ofaros LLC is proud to soon offer a thorough, vetted diagnostic evaluation option to assess Attention Deficit Hyperactivity Disorder (ADHD) and Autism Spectrum Disorder (ASD) here in Hueytown, AL for kids ages 5-18. This evaluation is made up of two parts. Phase 1 is meant to assess for ADHD and screen for ASD. In cases where Phase 1 indicates possible ASD traits, clients may proceed to Phase 2 to be assessed for ASD. At the end of the process, Ofaros will provide a full diagnostic report that includes all assessment results, clinical interpretations, diagnostic information, and potential treatment options and resources. Ofaros does not accept insurance for this service at this time.

Evaluation Process: 

Phase 1 (ADHD)

​

Step 1 – Intake Session (Week 1)

  1. This 90-minute session will include:

    1. Review of submitted intake paperwork.

    2. Sign the Release of Information forms that will be required to complete the various portions of this process.

    3. Taking client history and completing a Biopsychosocial interview.

    4. Discussing the process, timeline, and answering any questions.

  2. The intake session fee is $200, which is paid when scheduling.

 

Step 2 – Initial Assessments (Weeks 2 & 3)

  1. This step includes the completion of the following assessments:

    1. BASC-3 (Behavior Assessment)

    2. Brown EF/A (Executive Functioning and Attention Scales)

    3. ABAS-3 (Adaptive Behavior and Life Skills Assessment)

    4. SRS-2 (Social Responsiveness Scale)

    5. TOVA-9 (Test of Variables of Attention)

  2. Some of these Instruments will be sent to selected graders and cannot be reviewed until all submission have been completed and received.

  3. The total fee for these assessments and interpretations is $550 (paid at step 1 session).

 

Step 3 – Evaluation Report and Strategy Session (Week 4)

  1. This session will review the comprehensive report created from the assessments completed in Step 2, including any potential diagnosis and/or prognosis of symptoms.

  2. This session will also discuss potential treatment plans and any available resources depending on the findings.

  3. The fee for this session is $200 and lasts 90 minutes (paid at scheduling).

Phase 2 (ASD)

​

Step 4 – ASD Evaluation and Assessment (Weeks 5 & 6)

  1. This step includes the following assessments:

    1. ADOS-2 (Autism Diagnostic Observation Schedule)

    2. WISC-5 (Wechsler Intelligence Scale for Children – Ages 6-16); If older than 16, a different IQ assessment will be completed.

  2. The total fee for these assessments and interpretations is $900 (paid at step 3 session).

  

Step 5 – Evaluation Report and Strategy Session (Week 7)

  1. This session will include a review of the assessments complete, the interpretation and significance of the results, and potential treatment options.

  2. If applicable, a list of resources for collaborative and/or useful services will be provided.

  3. If applicable, a treatment plan will be discussed, including any at-home interventions, school setting recommendations, and referral to other professionals.

  4. The fee for this session is $200 and it lasts 90 minutes (paid at scheduling).

How to book (Coming Early 2024)

Step 1

Complete the Evaluations Intake Questionnaire by clicking HERE.

Step 2

Submit initial screening documentation HERE. 

Include: a. Letter from Eye Doctor regarding any vision issues (or lack of). b. Letter from your primary doctor regarding any auditory issues (or lack of). c. Referral from primary care doctor ruling out any physiological issues that may contribute to the behaviors of concern. d. Any previous psychological assessments and/or related documents.

Step 3

Send an email confirming that you have completed steps 1 and 2 to seth@ofaros.com. Please include your full name, child's full name, and the email address and phone number of the primary contact for the child's file.

Step 4

Within a few days of completing step 3, you will receive an email to create an account via our client portal. Complete the assigned documentation and schedule your intake session. Please allow 90 minutes for this session. You must have a credit/debit card on file for your booking to be accepted and you'll be charged the intake fee upon acceptance of the scheduled session.

Step 5

Arrive at your intake session 15 minutes before your scheduled appointment time. Please bring any documents or prior evaluations that you'd like to discuss.
